Evergreen Analytical Laboratory performs services in accordance with normal professional standards for the industry. The total liability of EAL, its officers, agents or employees, arising out of or in connection with the services provided, shall not exceed the invoiced amount. CONFIDENTIALITY: EAL maintains the strictest confidentiality in all of our client interactions. Laboratory results are retained five (5) years. They are maintained as the confidential property of our clients. Release of reports must meet both our approval and that of our clients. If you (or your client) require a signed confidentiality agreement, EAL will gladly comply.

COMPOSITING/SPLITTING SAMPLES: A fee will be charge for compositing or splitting samples prior to analysis.

COURIER POLICY: UPS Ground and Economy Courier Service are included in the listed prices on orders over $400. Clients are responsible for all return and any expedited delivery charges unless other prior arrangements have been made. The couriers used by EAL are independent contractors. We are not responsible for anything that happens to the samples in transit to the laboratory. Our responsibility begins once we take custody at our laboratory.

CREDIT TERMS: Payment in advance is required for all clients except for those whose credit has been established with EAL. Invoices are due upon receipt. VISA and MasterCard are accepted. Invoices not paid within 30 days may be assessed 1.5% interest per month. In the event of default on payment the client is responsible for all reasonable collection and legal fees.

DATA DELIVERABLES: Standard reports consist of hard copies of sample results, associated method blanks and surrogate recoveries. Several levels of additional Quality Control (QC) packages and electronic deliverables are available by quotation. Extended turnaround times may be required for some data packages.

DISCOUNTS: EAL fees are meant to be used as a guideline applicable to small sample lots. Please feel free to contact a Client Services Representative for a project specific quotation at any time.

MINIMUM CHARGE: The minimum billing charge is $75 per work order. Rush charges do not apply to reaching the minimum. Surcharges will be added to the minimum billing where the client requires an expedited turnaround. Discounts do not apply to minimum billing.

MULTIPHASE SAMPLES: Samples consisting of more than one phase will have each phase processed and invoiced as a separate sample, unless accompanying instructions specify otherwise.

ORGANIC MATRICES: A $25 surcharge per sample per test will be assessed for organic matrices (oil, oily sludge and organic waste), except for metals tests. A $75 microwave digestion fee will be assessed per sample for metals analyses.

QA/QC: Quality Assurance/Quality Control practices are applied to all methods employed. Method blanks are used to evaluate possible sources of system contamination and laboratory background. Other QC practices may include sample duplicates, matrix spike and matrix spike duplicates (MS/MSD), reference samples, and surrogate standards. Client specified MS/MSDs and/or Trip Blanks provided for volatile organic analyses are analyzed upon request, and will be invoiced as additional samples.

SAMPLE DISP0SAL/RETURN: All samples remain the property of the client. Certain hazardous samples, and samples placed on hold may be returned to the client upon completion of the work order. A minimum $50 surcharge will be added to orders requiring special packaging and labeling as hazardous materials. Applicable shipping charges will also be invoiced.
Samples not returned will be disposed of one (1) month from the report date.

TURNAROUND: The standard turnaround time for most analyses at EAL is 10 working days. Underground Storage Tank projects are given a priority three-day standard TAT due to the nature of UST work. Rush turnarounds are available for all tests subject to laboratory capacity and method requirements. Surcharges apply as follows:
